Parents,
Those students who are interested in sports at Charleston Middle School (CMS) must have a physical before tryouts next year. I would encourage you to be proactive and get a sports physical over the summer and turn it into the CMS office this summer. Below is an opportunity for you to get a sports physical for $20. A sports physical is good for one year.
Mark your calendar! Sarah Bush Lincoln's $20 sports physicals are coming up.
•The cost is $20 per physical with all fees being donated to the participating school’s athletic booster clubs.
•Physicals at Neoga are free to students in the Neoga school district.
•Immunizations will not be given at the clinics.
•Cash or check only will be accepted.
•Insurance will not be billed, nor will insurance co-pays be collected.
•Parent must sign medical history form or be in attendance.
•Appointments required.
•Schedule and details available at: https://www.sarahbush.org/sportsphysicals/
