The State of Illinois requires every child entering pre-school to meet specific medical requirements. These include a physical exam, lead assessment by his/her doctor, and up-to-date immunizations. The lead assessment is a more recent requirement. The date and result of the assessment must be turned in along with your child’s physical exam. A dental exam is strongly recommended. Every child entering school is required to present a copy of their certified birth certificate when registering for school. Please note the health exam form is two-sided. The top half of the backside of the form needs to be filled out and signed by the parent.
The following are the required immunizations for school attendance:
Measles, mumps, and rubella vaccine (MMR): One dose on or after 12 months of age. |

DPT/DtaP: Three doses by 12 months of age. One additional dose is required by 24 months of age. |
Polio: Two doses by 12 months of age. One additional dose is required by 24 months of age. |
Hib: One dose on or after 15 months of age. |
Hepatitis B: Series of three shots for all children 2 years of age and older. |
Varicella: One dose of chickenpox vaccine (varicella) on or after their first birthday. A physician can confirm past disease history in lieu of vaccination by having examined the infected child’s history or reviewing laboratory evidence. |
Your child can be immunized by his/her doctor or at the Coles County Public Health Department (217-348-0530). Make your appointments early so you don't get caught in the last-minute rush! The physical may be dated one year prior to the first day of school. Completed physicals with lead assessment and immunizations should be brought to central registration in early August. These health requirements MUST be turned in before the student can begin school.
