STUDENTS TRANSFERRING FROM ANOTHER ILLINOIS SCHOOL DISTRICT AND STUDENTS TRANSFERRING FROM OUT OF STATE
The School Code of Illinois, Section 5/27-8.1 requires students to have a physical exam with up-to-date immunizations upon entering kindergarten, sixth grade, and ninth grade. A more recent requirement is that all students have a dental exam upon entering kindergarten, second grade, and sixth grade. A vision exam is required for all new students to Illinois schools. All Illinois students must present a completed Illinois State Board of Education student transfer form from their former Illinois public school district.
Students transferring from out of state need to have the physical, vision, and dental exams completed using the forms on this page. Immunizations are given at the Coles County Health Department (217-348-0530) as well as in some physicians’ offices. All new students have 30 days to comply with this state requirement.
All students transferring into CUSD#1 must also provide a copy of their birth certificate.
FAILURE TO MEET THE ABOVE REQUIREMENTS WILL RESULT IN THE STUDENTS BEING EXCLUDED FROM SCHOOL UNTIL SAID REQUIREMENTS ARE MET. PARENTS/GUARDIANS NEGLIGENT IN ASSISTING THEIR SONS/DAUGHTERS IN MEETING THESE REQUIREMENTS WILL CAUSE THEM TO BE CONSIDERED TRUANT FROM SCHOOL.
